Ø  Original Passport with at least 6 months validity from date of travel to Singapore along with the old passports (If it has Previous Travel History to Singapore)

Ø  Duly-Filled Form (Handwritten only) 14A with passenger’s Signature (Form Must be filled only by the Passenger without any correction or whitener mark) For cases of passport address different from current address, it is sufficient to reflect the current address in Form 14A. Photo to be affixed in the form & cross signed by the passenger (Refer Attachment)

Ø  2 colored photographs with white background, 35x45mm in size taken in the last 2 months with face covering 80% of the photo area in Glossy Finish

Ø  Original Covering Letter from the Company indicating the purpose of travel,  Passport No, travelling companions (to list name and passport number) Travel Dates, Monthly/Annual Income details & contact no (Refer attachment for format)

Ø  Salary slips for the last 3 months  

Ø  Last 3 months bank statement of the passenger with sufficient balance

Ø  Letter of Invitation or V39 from Singapore company

Ø  VTP copy 

Ø  Copy of Vaccination Certificate (Covishield or Covaxin) with 14 days completed after 2^nd Dose

Ø  Travel Insurance with minimum coverage of 30000SGD for Covid Related expenses

Ø  Attached Authorization letter to be filled, signed & sealed by the Agent/Branch
